PURPOSE:

To provide guidelines for appropriate usage and disclosure of individually identifiable Health Information (IIHI) for patients, which are in compliance with HIPAA Privacy Regulations.
To supply notification to patients regarding our Privacy Practices and the patient’s rights related to their own health information (IIHI).

Notice of Privacy Practices

POLICY:

1.Befitting You uses patient health information to obtain payment, provide treatment and services, and to evaluate and improve the patient’s quality of care. Befitting You is required by law to protect the privacy of its patients’ Individually Identifiable Health Information (IIHI), as well as to provide a notice to its patients of those privacy practices.
2.A copy of the Befitting You Notice of Privacy Practices will be available to all patients.
3.Befitting You reserves the right to modify or update its policies without prior notification to its patients.
4.The Compliance Officer will be responsible for governing all activities related to privacy regulations and managing the administrative duties related to the Befitting You Privacy Practices.

PROCEDURE:

1.All new patients will receive a copy of the Notice of Privacy Practices with their initial paperwork given at the time of the initial services rendered.
2.The current Befitting You Notice of Privacy Practices will be posted in company location.
3.Befitting You has the right to modify or update its policies without prior notification to its patients.
4.Patients may receive an updated Notice at any time by submitting a request in writing to the Compliance Officer.
5.In compliance with HIPAA regulations, the patient has a right to review and update their health information. Requests from the patient to review or modify this information must be made in writing to the Compliance Officer.
6.The patient also has the right to refuse to provide consent to Befitting You for use or disclosure of Individually Identifiable Health Information (IIHI), except in certain cases of emergency or where required by law. Requests by the patient to refuse disclosure must be submitted in writing to the Compliance Officer, and may preclude Befitting You from continuing to provide services.
7.Befitting You is required to review these requests, but is not bound legally to agree to them. In certain circumstances, the Compliance Officer has the right to refuse to accept these requests.
